TRT and Testicular Shrinkage - Is It Inevitable?

I asked this over on the hormone replacement section but didn’t get any replies, so I figured I’d try here. Is testicular atrophy basically something you have to accept if you’re on testosterone replacement therapy long term?

Also, what’s a typical weekly testosterone dose for TRT? Would something like Nolvadex be part of the protocol, or not really? I’ve heard some people use low-dose HCG regularly to help prevent shrinkage - does that actually work, or is this just one of those side effects you end up living with if you want the benefits of TRT?

I’m mostly asking from a cosmetic standpoint, since smaller testicles seem like one of the more obvious downsides.

it’s the one part of the whole process that makes you do a double-take in the mirror. For a lot of guys, it’s just the "tax" you pay for feeling like a human again, but you definitely don't have to just roll over and accept it if it bothers you.

HCG is definitely the answer here. It’s not just some bro-science myth—it actually works by tricking your body into thinking it’s still getting the signal to produce, so things stay full and functional. Most guys who care about the cosmetic side or keeping their fertility in the picture swear by it.

Most TRT protocols land somewhere between 100mg and 200mg a week. Anything way higher and you're basically doing a "mini-blast" year-round, which comes with its own headaches. Leave the Nolvadex for the guys coming off a massive cycle—it’s not a standard part of a long-term TRT cruise. If the shrinkage is a dealbreaker for you, just ask for the HCG. It’s an extra pin a few times a week, but it keeps everything looking like it’s supposed to!

honestly man the shrinkage is pretty much a given for most guys once you shut down your natural production but it definitely does not have to be permanent if you stay on top of it. a typical trt dose is usually somewhere between 100mg to 200mg a week depending on your bloodwork and how you feel but everyone is a little different.

yeah if you’re on straight trt with nothing else then yeah shrinkage kinda just happens over time, some guys more than others. it’s not instant but you’ll notice it eventually. hcg does actually help though, not bro science. low dose a couple times a week can keep things looking pretty normal for a lot of people
